H & P Country Store (Bethany) – Routine inspection conducted on 3/8/23.

No critical or non-critical violations noted at time of inspection.

 

Lips to Hips (Bethany) – Routine inspection conducted on 3/8/23.

Critical – No critical violations noted at time of inspection.

Non-critical – Microwave is dirty. Clean.

Amana refrigerator is dirty. Clean.

Ice scoop is stored on top of ice machine. Store scoop in a bin or in ice with the handle up.

No paper towels in employee restroom. Restrooms must be adequately supplied.

Unnecessary clutter in back room. Clean.

Food packaged on site is not labeled. All food must be labeled.

 

Lips to Hips – Follow up inspection conducted on 3/15/23.

All critical and non-critical violations have been corrected.

 

The Café (Cainsville) – Routine inspection conducted on 3/15/23.

No critical or non-critical violations noted at time of inspection.

 

Taco Bell (Bethany) – Routine inspection conducted on 3/22/23.

No critical or non-critical violations noted at time of inspection.

 

Sonic Drive-In (Bethany) – Routine inspection conducted on 3/22/23.

Critical – No critical violations noted at time of inspection.

Non-critical – Ceiling vents are dirty by employee restroom. Clean.

Missing ceiling tiles in kitchen. Repair.
