Dairy Queen (Bethany) – Routine inspection conducted on 3/12/25.

Critical – Unlabeled spray bottle in store room. All toxics must be labelled. Corrected.

Potentially hazardous foods not date marked in prep cooler & walk in cooler. Date when made and discard after 7 days.

Non-critical – Back of toilet is broke in women’s restroom. Replace.

Excessive trash around dumpster & dumpster has not lid. Remove.

Unshielded bulbs in basement. All lights must be shielded in food storage areas.

Improper storage of mops. Mops must be stored so they can completely dry between uses. Corrected.

Bottom of sundae machine is dirty. Clean.

Sheet rock is crumbling & in disrepair by blizzard machine. Repair.

Cup dispenser above sundae machine is in disrepair & held together with tape. Surface should be smooth and easily cleanable.

No soap or paper towels at hand sink by walk in cooler. Hand washing station must be adequately supplied.

Floor of walk-in freezer is dirty. Clean.

Handle on fryer basket has duct tape on it. Equipment must be smooth and easily cleanable.

Plumbing is leaking at bottom of stairs in basement. Repair.

Single service items stored on floor & under sewer pipes in basement. Store 6” off of floor to prevent contamination.

 

Woods & Main (Eagleville) – Routine inspection conducted on 3/19/25.

Critical – No critical violations noted at time of inspection.

Non-critical – Vents in ceiling are dirty throughout dining area. Clean.

 

The Café (Cainsville) – Routine inspection conducted on 3/19/25.

Critical – No critical violations noted at time of inspection.

Non-critical – Whirlpool freezer is frosted over. Defrost & Clean.

 

Toot-Toots (Bethany) – Routine inspection conducted on 3/26/25.

Critical – No critical violations noted at time of inspection.

Non-critical – Single service items stored on floor of store room. Store 6” off of floor to prevent contamination.

 

Dollar General (Bethany) – Routine inspection conducted on 3/26/25.

Critical – No critical violations noted at time of inspection.

Non-critical – Insulation falling down around A/C unit. Repair.
