Lips to Hips (Bethany) – Follow up inspection conducted on 6/5/24.

All critical and non-critical violations have been corrected.

 

Hy-Vee (Bethany) – Routine inspection conducted on 6/5/24.

Critical – Dented and bulged can of fruit cocktail. Discard. Corrected.

Non-critical – No non-critical violations noted at time of inspection.

 

Huffman BBQ (Bethany) – Routine inspection conducted on 6/12/24.

No critical or non-critical violations noted at time of inspection.

 

Eagleville Oil – Routine inspection conducted on 6/12/24.

No critical or non-critical violations noted at time of inspection.

 

Stateline Conoco (Eagleville) – Routine inspection conducted on 6/12/24.

Critical – Ice machine is dirty. Clean.

Non-critical – Freezer is frosted over. Defrost so it’s smooth and easily cleanable.

Damaged ceiling tiles in kitchen. Replace.

Outdated children’s medicine. Discard. Corrected.

Blue Bunny freezer is frosted over. Defrost.

 

Neat’s Burger Barn (Eagleville) – Routine inspection conducted on 6/12/24.

No critical or non-critical violations noted at time of inspection.

 

Dairy Queen (Bethany) – Routine inspection conducted on 6/26/24.

Critical – Shelf above 3 compartment sink is dirty that holds spatulas. Clean & Clean utensils.

Several dead bugs in basement. Clean.

Unlabeled spray bottle. All toxics must be labelled.

Ketchup dispenser is dirty. Clean.

Non-critical – Back of toilet is broke in women’s restroom. Repair.

Area behind kitchen prep cooler is dirty. Clean.

Single service items stored on floor. Store 6” off of floor to prevent contamination.

Seal on cake freezer is moldy. Clean & sanitize.

No paper towels at hand sink. Hand sinks must be adequately supplied.

Vent is dirty by walk in cooler. Clean.

Area around front prep cooler is dirty. Clean.

Bottom of burger freezer is dirty. Clean.

Cloth and cardboard covering shelves in kitchen. Remove. Shelving must be smooth and easily cleanable & nonabsorbent.

Mops not stored properly. Mops must be stored where they can completely dry between uses.

Rolling cart in basement is dirty. Clean.

Several shelves are rusty in basement. Strip and re-paint.

 

McDonalds (Bethany) – Routine inspection conducted on 6/26/24.

Critical – No critical violations noted at time of inspection.

Non-critical – Large amount of ice build up in walk in freezer. Defrost & Clean.

 

Poppin Tops (Blythedale) – Routine inspection conducted on 6/26/24.

No critical or non-critical violations noted at time of inspection.

 

El Nopal (Bethany) – Follow up inspection conducted on 6/26/24.

All critical and non-critical violations have been corrected.
