Toot-Toots (Bethany) – Routine inspection conducted on 5/15/24.

Critical – Dented & crushed can of sauerkraut. Discard. Corrected.

Dented & crushed can of pears. Discard. Corrected.

Non-critical – Bottom shelf of prep table by oven is rusty. Strip and re-paint.

 

Pizza Hut (Bethany) – Routine inspection conducted on 5/15/24.

Critical – No critical violations noted at time of inspection.

Non-critical – Top of pizza oven is dirty. Clean.

Booths on north wall have tears in them. Repair.

 

Lips to Hips (Bethany) – Routine inspection conducted on 5/22/24.

Critical – No critical violations noted at time of inspection.

Non-critical – Bananas stored on floor. Store 6” off of floor to prevent contamination. Corrected.

No self-closing door on restroom. All restroom doors must be self-closing.

Oven is dirty. Clean.

No paper towels at hand sink. All hand sinks must be adequately supplied.

 

El Nopal (Bethany) – Routine inspection conducted on 5/29/24.

Critical – Eggs stored above limes & lemons in walk-in cooler. Eggs must be stored below ready-to-eat food to prevent contamination. Corrected.

Non-critical – Missing ceiling tile by Coke machine. Replace.

Damaged ceiling tile above ice machine. Replace.

Sink faucet is leaking by dishwasher. Repair.

Plunger missing off door of walk-in cooler. Repair.

Floor is damaged in walk-in freezer. Repair.

Bottom shelf of table with can opener is rusty. Strip & re-paint.

Mechanical ventilation is loose from ceiling in men’s restroom. Repair.
