Dairy Queen (Bethany) - Routine inspection conducted on 5/3/17.

Critical- Dead mouse under shelving in basement. Remove and clean basement.

Non-critical- Bottom of stainless prep cooler dirty, clean on routine basis. Corrected.

Unshielded bulbs above single service items behind ice machine, all lighting must be shielded.

Single service items stored on floor in basement, single service items must be stored 6” off of floor to prevent contamination.

Unnecessary clutter of empty boxes in basement, remove and clean.

Plumbing pipes leaking by the door going into basement, repair.

 

Sportsman’s Hut (Ridgeway) - Pre-opening inspection conducted on 5/3/17.

No critical or non-critical violations noted at time of inspection. This establishment is approved to open.

 

Eagleville Oil- Routine inspection conducted on 5/10/17.

Critical- No critical violations noted at time of inspection.

Non-critical- Mops not properly stored after using. Store so mop heads can completely dry in between uses.

Bottom of walk-in cooler dirty on side with display doors, clean on routine basis.

Broken door hinge under single service station by fountain machine, repair.

 

Subway (Bethany) - Routine inspection conducted on 5/10/17.

Critical- No critical violations noted at time of inspection.

Non-critical- Bag of bread stored on floor, store 6” off of floor to prevent contamination. Corrected.

Top of bread storage dirty, clean on routine basis.

 

Hope Royale (Bethany) - Routine inspection conducted on 5/17/17.

Critical- Can opener dirty, clean on routine basis. Corrected.

Non-critical- No soap at hand washing sink, hand washing sink shall be adequately supplied. Corrected.

No test kit for sanitizer, test kits must be readily available to test concentration of sanitizer.

Insufficient sanitizer in wash cloth bucket, sanitizer shall be between 50-200 ppm. Corrected.

Potatoes stored on floor in store room, store 6” off of floor to prevent contamination. Corrected.

 

Sharp’s Amoco (Bethany) - Routine inspection conducted on 5/17/17.

Critical- Unlabelled spray bottle in store room, all toxics shall be labeled. Corrected.

Gravy not date marked in cooler. All ready to eat potentially hazardous foods must be dated when made and discarded after 7 days. Corrected.

Non-critical- Missing ceiling tiles by door of walk-in cooler. Physical facilities shall be maintained in good repair.

 

Toot-Toots (Bethany) - Routine inspection conducted on 5/24/17.

Critical- Can opener dirty, clean on routine basis. Corrected.

Several food items in walk-in cooler uncovered with the risk of cross contamination, all food must remain covered. Corrected.

Several ready to eat food items not date marked in walk-in cooler. Date when made and discard after 7 days. Corrected.

Non-critical- Wash cloths not stored in sanitizer in between uses. Wash cloths must be stored in sanitizer when not using. Corrected.

Employee’s drink does not have a lid on it. All drinks must have a lid when in food prep areas. Corrected.

Food stored on floor of walk-in cooler and freezer. Store 6” off of floor to prevent contamination.
