March Inspections

 

Ridgeway R-5 School—Routine inspection conducted on 3/2/16.

Critical- Sanitizer too strong, quaternary solutions shall be mixed at a concentration between 200-400 ppm. Corrected during inspection

Can opener dirty, clean on routine basis, corrected during inspection.

Non-critical- Outer openings unprotected under double doors going to playground, repair.

 

Cainsville R-I School—Routine inspection conducted on 3/2/16.

Critical- Improper storage of toxics (fly spray) around food related items, remove. Violation was corrected during inspection.

Mice feces found on white shelves on east side of store room, clean and control pests.

Bottom of chest freezer, by refrigerator, in need of cleaning, all food contact surfaces shall remain clean.

Non-critical- Unshielded bulbs in storage room, all lighting shall be shielded.

Unnecessary clutter of boxes with moldy food piled in storage room, clean.

Single service items stored on floor, store 6” above floor to prevent contamination. Violation was corrected during inspection.

Gray rolling shelf at bottom of stairs in need of cleaning. Nonfood contact surfaces shall be clean to sight and touch.

 

Cainsville R-I School—Follow up inspection conducted on 3/9/16.

All critical and non-critical violations have been corrected.

 

North Harrison R-3 School—Routine inspection conducted on 3/16/16.

Critical- Raw proteins stored above ready to eat foods, store below ready to eat foods to prevent contamination.

Mice feces seen in store room, staff is aware of problem and working to correct it.

Non-critical- Ice machine appears to be leaking from the back, repair.

Outer openings unprotected on east wall by exit door, repair.

Exhaust fans and A/C ducts are dirty, clean on routine basis.

 

Dairy Queen (Bethany)—Follow up inspection conducted on 3/23/16.

All critical and non-critical violations have been corrected.

 

Camaro’s Steakhouse (Bethany)—Routine inspection conducted on 3/23/16.

Critical- Ready to eat potentially hazardous foods are not date marked. All PHF’s shall be dated when made and discarded after 7 days. Violation was corrected during inspection.

Non-critical-No towels or dryer at hand sink by soda machine. Hand washing facility shall be adequately supplied.

Bottom of walk in cooler dirty, all non-food contact surfaces shall remain clean to sight and touch.

No covered waste basket in women’s restroom, all wastebaskets in women’s bathrooms shall be covered.

 

Gilman City R-IV School—Routine inspection conducted on 3/30/16.

Critical—No critical violations were noted at time of inspection.

Non-critical-Mops not stored properly, mops shall be stored so they can air dry.

 

Camaro’s Steakhouse—Follow up inspection conducted on 3/30/16.

All violations have been corrected.
