The following inspections were conducted during the month of March 2015:

South Harrison Elementary
(Bethany) – Routine inspection conducted on 03-05-15.
Critical
(1) Inside of microwave contains food residue and splash. Food contact surfaces shall be washed, rinsed and sanitized as often as necessary to keep them clean. Violation was corrected during inspection.
(2) Three unlabeled squirt bottles. Bottles shall be labeled as to contents. Violation was corrected during inspection.
Non-critical
Three violations including: 1. Outside of microwave in need of cleaning (corrected); 2. Tupperware containers have rough inner surfaces (discarded); 3. Floor under center pallets in store room in need of cleaning, edges of filters in stove hood in need of cleaning.

 

Dinner Bell (Eagleville) – Routine inspection conducted on 03-09-15.
Critical
(1) Employees unable to answer questions regarding internal temperature requirements for chicken and ground beef, one was unable to locate cook’s thermometer. Employees shall be knowledgeable regarding food safety and code.
(2) Raw eggs stored over biscuits in walk-in cooler. Raw protein foods shall not be stored over ready-to-eat foods to prevent cross-contamination.
(3) The following food contact surfaces are in need of cleaning: can opener, cappuccino chutes, toaster, cutting board of prep cooler at left end of line, bottom compartment of breakfast prep cooler. Food contact surfaces shall be washed, rinsed and sanitized as often as necessary to keep them clean.
(4) Vent hood edges and plugs are grease-laden and dripping onto grill. Food contact surfaces shall be protected from contamination.
(5) Unlabeled spray bottle. Spray bottles shall be labeled as to contents.

Eleven violations including: 1.Food items on floor of store room and walk-in; 2. Underside area of ware washing machine in need of cleaning; 3. The following non-food contact surfaces in need of cleaning: underside of upper shelves and bottom shelves in line area, inside and outside of plastic drawers containing utensils, outside of appliances and coolers, splash areas of soda and milk dispensers and malt machine, outside of waffle iron, underside of shelf over steam table, control knobs below steam table;
4. Shelves under steam table rusting, pitting, racks rusty in breakfast prep cooler; 5. Shelves under grill covered with foil, shelves in line area lined with paper, towel lining bottom of Turbo Air; 6. Handle missing on Turbo Air; 7. Floors, outlet covers, walls, area between ice machine and wall in need of cleaning, excessive ice build-up on floor of walk-in freezer; 8. Stained ceiling tiles and vents, floor tiles broken and missing, inside and outside doors of walk-in rusting, inside of back exit door rusting;
9. Personal items (gloves) laying on waffle prep table; 10. Wiping cloths not stored in sanitizer; 11. Gaps around back exit door.

(Eagleville) – Follow-up inspection conducted on 03-23-2015.

Eight violations (all repeat) including: 1. Underside areas of ware washing machine and drain boards in need of cleaning; 2. The following non-food contact surfaces are in need of cleaning: bottom shelves in prep/line area, splash areas of milk dispenser and malt machine; 3. Shelves under steam table rusty, pitting and covered with paper; 4. Handle missing on Turbo Air; 5. Floors, outlet covers, walls in need of cleaning; 6. Ceiling tiles stained, vents rusty and stained, floor tiles broken and missing, inside and outside of both walk-in doors rusty, back exit door rusty; 7. Wiping cloths not stored in sanitizer; 8. Gaps around back exit door.

Ridgeway R-5 School – Routine inspection conducted on 03-10-2015.

Four violations including: 1. Sanitizing solution weak in ware washing machine; 2. Underside of mixer has accumulation of food residue, side of oven and Blodgett have accumulation of grease and lint; 3. Mixer and bottom shelf of stainless table have areas that are chipping and rusting; 4. Door of walk-in freezer is rusting and walls on either side of door are in need of repair.

Cainsville R-1 School – Routine inspection conducted on 03-16-2015.

Three violations including: 1. Clean dishes too close to hand sink (corrected); 2. The following non-food contact surfaces no longer smooth and easily cleanable: hand sink edges not flush with counter, missing Formica in hand sink area, base of cabinetry in ware washing and hand sink area crumbling, bottom shelf of center prep table pitting, wood shelves in store room gouged and chipping; 3. Floors under shelves and equipment in store room in need of cleaning and paint is peeling.

 

 

Gilman City R-4 School – Routine inspection conducted on 03-17-2015.

Four violations including: 1. Wiping cloths lying out (corrected); 2. Personal food (student lunch bags) stored with school lunch food; 3. The following non-food contact surfaces in need of sanding, painting, sealing, or repair: shelves in walk-in freezer, some of the red shelves in store room (rusting), bottom metal shelf (rusting) and bottom laminate shelf in dish washing area (laminate missing), Formica on cabinet under microwave (chipping and taped), painted areas on mixer (chipping); 4. Floor in store room has profuse chipping.

South Harrison R-2 High School (Bethany) – Routine inspection conducted on 03-24-2015.

Three violations including: 1. Boxes of food products stored on floor of both walk-ins; 2. The following non-food contact surfaces are in need of sanding, painting, sealing or repair: mixer (chipping), backsplash of stove top and side of Blodgett (chipping, rusting), cabinet under microwave (chipping), bottom shelves of some of the rolling carts (rusting), wooden table under ice machine (unsealed);
3. Floor edges behind ware washing machine and areas of wall in need of cleaning.
