The following inspections were conducted during the month of July 2015:

Hy-Vee # 1036 (Bethany) – Routine inspection conducted on 07-07-15.

Five violations, including: 1. Bananas stored on floor (corrected); 2. Fan guards in walk-in cheese cooler are in need of cleaning, there is some debris and food items under retail shelves and ice build-up and debris under shelves of walk-in ice cream freezer; 4. Personal food stored with retail food items in dairy walk-in cooler (corrected); 5. Ceiling in warehouse over “Aisle 5” items is stained and bulging, some ceiling tiles in meat area are stained; 6. There are gaps around back door and seals around door and frame are in disrepair.

Selena’s Café (Bethany Country Club) – Routine inspection conducted on 07-20-15.
Critical
(1) Both small Haier coolers are above 41.0 F. Cold foods shall be held at 41.0 F or below. The temperatures of the coolers were adjusted during the inspection, rechecked, and the violation was corrected during the inspection.
(2) One cutting board is in need of cleaning. Food contact surfaces shall be washed, rinsed, and sanitized as often as necessary to keep them clean. Violation was corrected during the inspection.
(3) Cleaning supplies stored on counter top with food items. Only products necessary for cleaning and approved for use in a food establishment kitchen shall be present and all toxics shall be stored separately and so as not to contaminate food, utensils, and food related items. Violation was corrected during the inspection.

Nine violations, including: 1. No paper towels at hand sink (corrected); 2. Bag of rice stored on kitchen floor (corrected); 3. Sanitizer strength in third compartment of sink is at 0 ppm (corrected); 4. No test strips to test sanitizer strength of quaternary ammonia sanitizer (corrected); 5. Outer surfaces of some cabinetry and one refrigerator in need of cleaning (corrected); 6. Storage closet is disorganized and cluttered; 7. Wiping cloths not stored in sanitizing solution (corrected); 8. No thermometer in one cooler (corrected); 9. Cup used as scoop stored on top of ice (corrected).

China King (Bethany) – Routine inspection conducted on 07-27-15.
Critical
(1) Can opener is in need of cleaning. Food-contact items shall be washed, rinsed, and sanitized as often as necessary to keep them clean. Violation was corrected during the inspection.
(2) STP, Rustoleum, and paint are stored in kitchen under hand sink. Only items necessary for cleaning and approved for use in food establishments shall be present. Violation was corrected during the inspection.
(3) Bait station in store room is not covered. Only covered bait stations shall be used. Violation was corrected during the inspection.
(4) Clean trash bags are being used to cover food items. Only food-grade containers and materials shall be used for storing or covering food.

Nine violations, including: 1. Employee washing hands in ware-washing sink; 2. No thermometer in upright freezer; 3. Odor of sewage coming through drains; 4. Wiping cloths not stored in sanitizing solution (corrected); 5. No wastebasket at hand sink; 6. Sanitizer strength in wiping cloth buckets is
0 ppm for chlorine sanitizer (corrected); 7. Several non-food contact surfaces are in need of cleaning;
8. Single-service items for take-out are stored improperly (corrected); 9. Ceiling tiles over water heater are stained.
A follow-up inspection will be conducted.

El Nopal (Bethany) – Routine inspection conducted on 07-28-15.
Critical
(1) Employees are not washing their hands. Employees shall wash their hands before donning gloves, after using the restroom, between tasks, after handling soiled equipment or utensils, after touching self, after coughing, sneezing, using a tissue, smoking, eating, drinking, or engaging in other activities that may contaminate hands.
(2) Raw eggs stored over lemons and limes in walk-in cooler. Raw protein foods shall not be stored over ready-to-eat foods to protect from cross-contamination.
(3) Tubs of sauces, cheese, tomatoes, and tomato juice are setting on the floor in the middle of the kitchen traffic area and are uncovered. Food items shall be stored off the floor and covered to prevent contamination.
(4) The following food-contact surfaces and/or items have food residue build-up, mold, or are in need of cleaning: can opener (very dirty), knives and sharpener over can opening area, inside of large ice machine, ice scoop. Food contact surfaces and items shall be washed, rinsed and sanitized as often as necessary to keep them clean.

Twelve violations, including: 1. No wastebasket at hand sinks in kitchen and bar; 2. Hand sink in kitchen is dirty; 3. Food on steam table, food items in walk-in cooler uncovered; 4. Canned goods stored on floor by can rack; 5. Prep cooler not maintaining temperature; 6. Cardboard lining shelf at fryer; 7. Bottom shelves of two prep tables are rusting and pitting; 8. There are gaps around both back doors;
9. Employee restroom has strong odor of sewage, no soap or towels, there is no hot water in women’s restroom; 10. Many broken or cracked floor tiles, ceiling tiles stained in several areas; 11. Wiping cloths (several places) are not stored in sanitizing solution; 12. Thermometer is broken in bottom of prep cooler.
