December 2015 Inspections

 

Sharp’s Amoco (Bethany)—Routine inspection conducted on 12/2/15.

Critical- Unlabeled spray bottle in storage room, all toxics shall be labeled.

Ready to eat potentially hazardous foods not labeled or dated. All PHF’s shall contain a label and date.

Both critical violations were corrected at time of inspection.

Non-critical-Ice bagged on site not labeled. Ice bagged on site shall be labeled with name and address of store or store number.

Unshielded bulbs above shelf in store room, all lighting shall be shielded above food storage areas.

Insufficient sanitizer in sanitizer bucket, sanitizer shall be between 100-400 ppm for Quaternary solution and 50-200 ppm for chlorine. Violation was corrected during inspection.

Boxes stored on floor in store room. All food products shall be stored 6” above the floor to prevent contamination. Violation was corrected during inspection.

No soap at hand sink, all hand washing facilities shall be adequately supplied.

 

Subway Store #58056 (Eagleville)—Routine inspection conducted on 12/2/15.

Critical-No critical violations were noted at time of inspection.

Non-critical-Three compartment sink plugged causing incorrect order of wash/rinse/sanitize.

Single service items stored on floor, single service items shall be stored 6” above the floor to prevent contamination. Violation was corrected at time of inspection.

 

State Line Conoco (Eagleville)—Routine inspection conducted on 12/2/15.

Critical-Ready to eat potentially hazardous foods not dated. Date when made and discard after 7 days.

Non-critical-Ice bagged on site not labeled, bags shall contain store name and address or store number.

Unshielded bulbs in kitchen, all lighting shall be shielded above food prep areas.

No hot water at hand sink (handle broken), hand washing facilities shall be maintained in good condition.

Ceiling tile in disrepair above hand sink, physical facilities shall be maintained in good condition.

No self closing door on restroom, all restrooms shall have self closing doors.

Food stored on floor in kitchen, all food shall be store 6” above floor to prevent contamination.

Single service items stored on floor of warehouse, all single service items shall be stored 6” above floor to prevent contamination.

Vent above soda fountain is dirty, clean on routine basis.

Thermometer missing from milk cooler, walk-in cooler and west refrigerator in kitchen, all cooling units shall contain a thermometer.

 

Subway Store #58056 (Eagleville)—Follow up inspection conducted on 12/9/15.

All non-critical violations have been corrected.

 

Dinner Bell (Eagleville)—Routine inspection conducted on 12/9/15.

Critical-Ready to eat potentially hazardous foods not dated. Date when made and discard after 7 days.

Three unlabeled spray bottles throughout establishment, all toxics shall contain a label.

Can opener dirty, all food contact surface shall remain clean.

All critical violations were corrected at time of inspection.

Non-critical-Air vents dirty above prep area in kitchen, clean on routine basis.

No paper towels at hand washing sink, violation was corrected during inspection.

Food stored on floor of walk in freezer. All food shall be stored 6” above floor to prevent contamination.

Several containers of food uncovered in walk in cooler. Food containers shall remain covered when not in use.

Damaged and cracked floor tiles throughout kitchen, physical facilities shall be maintained in good condition.

Walk in freezer door has build up of ice around it, preventing it from fully closing. Physical facilities shall be maintained in good condition.

 

Bethany Nutrition Site—Routine inspection conducted on 12/9/15.

Critical-Ready to eat potentially hazardous foods not dated.

Employee drinking from cup without lid, all employees’ drinks shall contain a lid.

Non-critical-Vent hood above oven is dirty, clean on routine basis.

Containers in walk in have mold and mildew on them. Discard

Hole in kitchen ceiling, all physical facilities shall be maintained in good condition.

 

H & P Country Store (Bethany)—Routine inspection conducted on 12/16/15.

Critical-Can dented on side seam, item was discarded during inspection.

Non-critical-No non-critical violations were noted at time of inspection.

 

Harrison County Food Pantry (Bethany)—Pre-opening inspection at new location conducted on 12/16/15.

Critical-Can dented on side seam, item was discarded during inspection.

Non-critical-No non-critical violations were noted at time of inspection.

 

McDonalds (Bethany)—Routine inspection conducted on 12/16/15.

Critical- Employees not washing hands before putting on gloves, wash hands then put on gloves. Violation was corrected during inspection.

Non-critical-Wiping cloths lying out in several locations throughout establishment, wiping cloths shall be stored in sanitizer solution between uses. Violation was corrected during inspection.

Ice scoop handle stored on ice in ice machine, scoop shall be stored so handle does not touch ice. Violation was corrected during inspection.

Damaged floor tiles by hot water heater, replace.

Missing ceiling tiles above food storage area and by fryers, physical facilities shall be maintained in good condition.

 

Stateline Conoco (Eagleville)—Follow up inspection conducted on 12/23/15.

All critical and non-critical violations have been corrected.

 

Eagleville Oil (Eagleville)—Routine inspection conducted on 12/23/15.

Critical—No critical violations were noted at time of inspection.
