April Inspections

 

North Harrison R-3 School-Follow up inspection conducted on 4/6/16.

All critical and non-critical violations have been corrected.

 

Shooter’s (Bethany)- Routine inspection conducted on 4/6/16.

Critical-Unlabeled spray bottle, all toxics shall contain a label. Violation was corrected during inspection.

Scented bleach used for sanitizer solution, only use unscented bleach for sanitizing. Violation was corrected during inspection.

Non-critical-Unshielded bulbs above food storage area, all lighting above food storage and food prep areas must be shielded.

 

Subway (Bethany)-Routine inspection conducted on 4/6/16.

Critical-No critical violations were noted at time of inspection.

Non-critical-Insufficient sanitizer in 3 compartment sink, quaternary solutions shall be between 200-400 ppm. Violation was corrected during inspection.

Top of bread cabinet and the floor of walk-in cooler are in need of cleaning. Non-food contact surfaces shall remain clean to sight and touch.

 

Firehouse 54 (Eagleville)-Routine inspection conducted on 4/12/16.

No critical or non-critical violations were noted at time of inspection.

 

Kum & Go (Bethany)-Routine inspection conducted on 4/12/16.

Critical- No critical violations were noted at time of inspection.

Non-critical- Single service items stored on floor in storage room by kitchen, store 6” off floor to prevent contamination.

Floor of walk-in freezer is dirty, clean on routine basis.

 

 

South Harrison R-2 Elementary-Routine inspection conducted on 4/20/16.

Critical- Inside of microwave is dirty, clean on routine basis. Violation was corrected during inspection.

Non-critical- Vent hood screens are dirty, clean on routine basis.

Missing ceiling tile in storage room above bread racks, repair.

 

South Harrison R-2 High School-Routine inspection conducted on 4/20/16.

Critical- Unlabeled spray bottle, all toxics shall contain a label. Violation was corrected during inspection.

Ready to eat potentially hazardous foods not date marked. Violation was corrected during inspection.

Non-critical- Food stored on floor of walk-in freezer, store 6” off floor to prevent contamination.

 

El Nopal (Bethany)-Routine inspection conducted on 4/27/16.

Critical- Unlabeled spray bottle in kitchen, all toxics shall contain a label. Violation was corrected during inspection.

Toxic items being used to sanitize equipment is not approved for use in a food establishment. Violation was corrected during inspection.

Ready to eat potentially hazardous foods not date marked in walk in cooler. Violation was corrected during inspection.

Tea dispenser nozzle is broke, food contact surfaces shall be maintained in good repair.

Potentially hazardous foods not stored at 41 degrees or below, thermostat was adjusted until correct temperature was met.

Non-critical-Wiping cloths not stored in sanitizer, always store wiping cloths in sanitizer solution when not in use.

Meat in tub thawing on floor with risk of contamination, thawing should be done under running water or in the cooler. Violation was corrected during inspection.

Food items uncovered in walk-in cooler, food shall remain covered to prevent contamination.

Unshielded bulbs in hallway by walk-in cooler and freezer, all lighting in food prep and food storage areas shall be covered.

No towels or wastebasket at hand washing sink in kitchen. Hand washing facilities shall be adequately supplied. Violation was corrected during inspection.

No covered wastebasket in women’s restroom. All wastebaskets shall be covered in women’s restroom.

Handle on urinal in men’s restroom sticks down causing it to continue to run. Restrooms shall be maintained in good repair.

No self closing door on employee restroom in kitchen. All restrooms shall have self closing doors.

Faucet on 3 compartment sink in kitchen is leaking. All plumbing shall be maintained in good repair.

Damaged ceiling tile above coffee pot, replace.

Coat stored on top of canned food rack. Personal items shall be stored separately from food items.
