North Harrison R-3 School
(Eagleville) – Routine inspection conducted on 04-06-15.

Non-food contact surfaces no longer smooth and easily cleanable, including inside of cleaning supply cabinet, shelves in store room, Formica on mixer table top, shelves and door in walk-in freezer.

(Bethany) – Routine inspection conducted on 01-13-2015.

(1) Eggs in Budweiser refrigerator are at 48.5 degrees Fahrenheit. Potentially hazardous foods shall be kept at 41.0 degrees F or below. Violation was corrected during inspection.
(2) The following food contact surfaces contain food residue, spillage, and/or splash and are in need of cleaning: inside of microwave very dirty (corrected), freezer and refrigerator of two-door stainless cooler (spillage, debris), inside of Woods upright freezer (debris and also must be defrosted), refrigerator compartment of Hot Point (spillage and debris), inside of Budweiser cooler (spillage in bottom). Food contact surfaces shall be washed, rinsed, and sanitized as often as necessary to keep them clean.
(3) Chili in Woods freezer and four containers of meat and a take-out container are not date-marked. Ready-to-eat, potentially hazardous foods shall be marked with a seven-day discard date and kept at 41.0 degrees Fahrenheit or below. Violation was corrected during inspection.

Thirteen violations including: 1. No wastebasket at hand sink (corrected); 2. No soap at hand sink (corrected); 3. Insufficient sanitizer in three-compartment sink (corrected); 4. Bar sinks are set up improperly (wash, sanitize, rinse) and must be wash, rinse sanitize (corrected); 5. Non-food contact surfaces are in need of cleaning (outside of toaster, shelf under microwave, outside of appliances especially handle areas, flour container, seasoning containers); 6. Contact paper is covering shelf holding microwave; 7. Several surfaces are in need of painting or sealing (bare wood areas in kitchen, bottom metal shelf of prep table in kitchen); 8. Several pieces of equipment are in need of repair (bar ice machine door, Deep Freeze chest freezer, GE upright, Woods, Hot Point); 9. Thermometers missing or inaccurate in two-door stainless, beige refrigerator, Budweiser cooler (corrected); 10. Unshielded appliance bulb in beige refrigerator; 11. Store room is cluttered and contains equipment no longer working; 12. Floor under two-door stainless cooler and oven and floor in storeroom in need of cleaning; 13. Wiping cloth lying out on prep table (corrected).

All priority violations have been corrected.

One violation, including Deep Freeze chest freezer and Woods upright freezer both have areas repaired with duct tape.


Selena’s Mobile Kitchen
– (Bethany) – Routine inspection conducted on 04-20-2015.
Critical
(1) Cooked chicken in Haier cooler is not date-marked. Ready-to-eat, potentially hazardous foods shall be marked with a seven-day discard date, if kept at 41.0 F or below. Violation was corrected during inspection.
(2) Raw eggs stored over open can of pineapple and other ready-to-eat foods in Haier cooler. Raw protein foods shall be stored below ready-to-eat foods to protect from cross-contamination. Violation was corrected during inspection.
(3) Water at hand and ware washing sinks is not working properly. No hot water. There shall be adequate and properly operating hot and cold water for hand and ware washing. (A temporary hand washing station was set up until the malfunction is repaired.)

Two violations including: 1. No hot water; 2. Thermometers missing from cooling units.
